Position Title : Resource Navigator Organization : Jewish Family Service of San Diego Department: Jewish Community Integration Services Position Type : Full-Time (37.5 hours/week), Non-Exempt Work Setting: Fully Onsite Reports To: Director of Aging Connections Pay Range: $25- $27/hour Total Compensation : In addition to standard pay, compensation for this position includes: Comprehensive, low-cost healthcare coverage for employees Generous employer 401(k) contributions Employer-covered life insurance Time Away from Work: Being able to take time away from work is critical in bringing your best self to work. Time off benefits for this position include: Paid vacation time and sick leave 15 paid holidays, including Federal and Jewish holidays (as long as the holiday lands on a normal scheduled work day), and floating holidays 2 Wellness Days to be taken any time during the year to support employees’ mental wellness Position Overview: JFS is looking for a compassionate, mission-driven individual to join the Jewish Community Integrated Services team as the Resource Navigator In this role, you have the opportunity to use your compassion and case management experience to make a meaningful difference in your community and help change lives across Orange County and San Diego. The Resource Navigator plays an integral role on the team and supports the entire agency by working closely with clients to assess their needs and provide assistance with resource navigation in Orange and San Diego County. This position reports directly to the Director of Aging and Resource Navigation and works closely with the Jewish Community Integration Services Team and the rest of the Nutrition and Aging Care Services Division. Responsibilities : Follow up on assigned referrals and assess each situation and screen for high-risk situations Carry a caseload of individuals and families for follow up (estimated 40 households) Provide comprehensive assessments for individuals and families, identifying areas of need and helping clients set goals Help clients identify goals related to financial stability, increasing income, increasing savings, employment, and housing stability. Document progress at regular intervals Ensure clients access public benefit programs for which they are eligible Provide appropriate external referrals and document progress at regular intervals Work closely with other case managers and service providers to coordinate care for shared clients Maintain documentation of client interactions in software system with integrity and in line with the quality standard set by supervisor Other duties assigned by supervisor Skills/Experience/Abilities That Are a Must-Have: Bachelor’s degree (in a human services field – social work, gerontology, psychology, etc.) Must have a valid CA driver's license, insurance, a reliable personal vehicle, and a willingness to travel off-site Skills/Abilities We’d Like You to Have : Culturally sensitive and ability to use a translator if needed Understanding of the low-income population and the concept of trauma informed care Ability to show an unconditional positive regard for clients who turn to JFS for support Familiarity with Jewish history, traditions, and culture Detailed-oriented and conscientious documentation due to contract standards Experience setting limits when working with stressed clients, often in some level of urgency Strong written and verbal communications skills Physical Requirements: Complete phone calls at desk, lasting from 5-60 minutes Be available for in-person appointments Enter documentation on the computer Utilize technology for meetings, phone calls, and data documentation Meet and greet clients on the campus and secure private settings to hold conversations Travel to other JFS worksites when assigned Other Duties: Please note that this job description is not designed to cover or contain a comprehensive listing of activities, duties, or responsibilities that are required of the employee for this job.
