Job Description:
A company is looking for a Care Manager Registered Nurse.
Key Responsibilities
Develop personalized treatment plans and support a
multidisciplinary team approach for health promotion
Assess members' needs and document progress in the electronic
medical record while collaborating with clinical teams
Facilitate Care Team meetings and provide education to members on
self-care and chronic condition management
Required Qualifications
Currently licensed Registered Nurse in the state of practice
Current certification in Basic Life Support for healthcare
providers
Current Wellness Coach Certification or equivalent, to be obtained
within 24 months of hire
Bachelor's degree (BSN) preferred
At least three years of nursing experience with chronic conditions
such as diabetes or heart disease
