Reporting to a well-tenured and experienced Medical Director, with
strong leadership and program management skills, the Transplant
Coordinator Nurse Practitioner will work on a small, tight-knit
team of Advanced Practice Providers, Cardiologists, RN Transplant
Specialists. This NP Leader will get to work with Nurse
Practitioners and Clinical Nurse Specialists who specialize in a
wide range of Pediatric Cardiac Care, including heart transplant &
failure, single ventricle physiology, inpatient cardiology care,
and post-surgical cardiac care.
The Coordinator Nurse Practitioner will have the opportunity to
work with with a wide range of inpatient hospital units, outpatient
clinical services, and external organizations while leading the
Transplant Program. In the 22-bed inpatient Cardiology / Pulmonary
Transplant Intensive Care Unit, this ACNP will find Nurse and
Physician specialists who focus on a diverse array of heart and
lung issues in both hospitalist and outpatient / clinic settings.
These clinical services also extend to pre-procedure assessment and
preparation, as well as post-operative outpatient follow-up and
Serving as both a Primary Provider for Transplant Patients, and as
Coordinator for the Transplant Program, the Nurse Practitioner will
actively engage in the full range of care.
This person will work help evaluate patients upon admittance,
working with Cardiology leaders and liaisons from other specialties
to develop an actionable diagnosis.
Following Organ Procurement and Transplantation Network (OPTN)
policy and procedure, the NP will identify and evaluate suitable
candidates to receive Transplantation. This includes data
collection, documentation, and reporting with organ procurement
Working in compliance with OPTN and CMS guidelines, the Transplant
Coordinator NP will consult with Cardiologists, Surgeons,
Pediatricians, and other Advanced Practice Providers to develop and
implement a transplant plan.
The NP Transplant Coordinator will serve as the Program Champion
and Educator, supporting education and development of health system
employees, as well as community and public education

person will have to opportunity to work with Hospital and national
quality leaders to organize and implement quality assurance and
performance improvement (QAPI) initiatives to ensure the programs
continues success. Pediatric NATCO certification is preferred for
the incoming Nurse Practitioner.
