Description:Assists an Area Home Health/Hospice Administrator in planning, developing, managing, delivering and evaluating/improving hospice nursing and other direct patient care treatment /clinical services provided in the home setting by a multi-disciplinary professional staff. Accountable to the Region and assigned Service Area for delivery of Hospice program(s) and service(s) to patient population. Ensures that quality, utilization, access, patient care and financial standards/goals are achieved or exceeded. Integrates patient care services with departmental, service line and organizational/strategic goals and objectives.Essential Responsibilities:Coordinates and supervises the delivery of hospice services, activities and patient care provided by an interdisciplinary staff/team in the home setting; formulates and monitors quality and performance criteria, policies procedures and service standards.Staff may include RNs, PHNs, Social Workers, Physical/Occupational Therapists and Aides.Establishes, implements, and maintains patient care and service standards to meet member service needs and expectations.Monitors and facilitates workflow, maximizing utilization of resources to provide the highest quality of care.Manages staff, provides clinical supervision and implements, delivers and --evaluates/ improves assigned programs and services.Ensures compliance in meeting all regulations and standards related to hospice, Medicare, TJC, Title 22, and other federal, state, and local requirements.Ensures accuracy and completeness of patient records.Identifies service delivery/quality issues and implements corrective actions/plans for improvement.Determines the appropriate staffing requirements and develops processes to interview, hire, train and maintain the competency of all department staff.Assesses professional development needs of staff and plans for training/continuing education of professional staff.Provides clinical leadership and support to patient care team.Serves as a role model for professional practices and development.Acts as patient advocate resolving patient care issues. Interprets and explains the program benefits to members and caregivers.May provide liaison, monitoring, consultative expertise, orientation and continuing education to contract agencies.Participates in special projects; attends and/or conducts meetings, committees in the department and medical center(s).Conducts a quality management/improvement/assurance program to ensure compliance with standards for hospice services/programs and external regulatory agency requirements.In collaboration with the Area Home Health/Hospice Administrator, Continuing Care Leaders and other appropriate medical and non-medical staff, evaluates clinical outcomes and performance of programs/services and makes appropriate recommendations for new modalities/programs or modification/improvement of existing programs.Ensures practices, services and operating procedures comply with administrative, legal and regulatory requirements of the Health Plan and governmental and accrediting agencies.May perform other duties as assigned.Must be able to work in a Labor/Management Partnership environment.Basic Qualifications:ExperienceMinimum three (3) years of supervisory/management experience required in a licensed Medicare certified hospice or home health agency, within the last five (5) years.EducationBachelor's degree in nursing, or Bachelor's in a health care related field.License, Certification, RegistrationCurrent RN license required; California RN license required by date of hire.Additional Requirements:Ability to demonstrate knowledge of and to apply state, federal and TJC standards and requirements regarding hospice operations, services and programs.Demonstrated ability to utilize written and verbal communication skills, as well as management skills such as problem solving, budget/program planning, performance improvement, organizational planning, implementing and monitoring performance standards.Must be able to work in a Labor/Management Partnership environment.Preferred Qualifications:Supervisory/management experience in an acute care hospital-based hospice agency.MSN or Master's in healthcare related field.
